Stay Healthy Latrobe (SHL) is a program which provides care coordination and self management support for people diagnosed with chronic or complex conditions. Care coordinators (Key Workers) work with consumers to: - Support and assist in the provision of relevant information regarding their condition.
- Assist in the development of care planning and goal setting for a healthier lifestyle.
- Coordinate services, access and appointments.
The SHL Care coordinator (Key Worker) liaises with the consumers' GP and their other health providers to assist with coordination of care.
People with chronic disease (with or without complex needs) who may progress toward requiring hospitalisation in the medium to long term. The consumer has; Chronic Disease (one or more of the following) - Diabetes-Type 1 or 2
- A Cardiovascular condition -such as Heart disease, Angina or Heart failure.
- A Respiratory condition-such as Asthma or COPD.
OR Complex/Multiple Service Requirements- Where the consumer requires acccess to multiple services that require coordination. - Two or more internal services
- Three or more services that are a combination of internal and external.
OR Intra-agency care plan eg. Team Care Arrangement (TCA) that requires coordination. Consumers who live outside the Latrobe LGA are encouraged to seek services in their own catchment area,however services cannot be denied on the basis of location if the consumer wishes to specifically access LCHS.
- SHL Care Coordinator (Key Worker)- NO COST
- Provider visit $8.30 except Podiatry $12.00
- Fees capped after 6 visits per calendar year
- Attendance at groups at regular cost price, not included in capped fee.
- Ability to waiver fee
Appointments made via SHL Care Coordinator from the Priority Waitlist in SWITCH. Monday - Friday: 9.00 am to 5.00 pm. - Churchill
