How do I move my aged care services to Latrobe Community Health Service?

We aim to make transferring your Home Care Package as easy as possible. We do this by connecting you with a dedicated member of our team who will take you through each step. There are two ways you can start the process:

  1. Call 1800 242 696. Press 2 for Home Care Services, then 2 again. You will be connected with a team member who will help you.
  2. Request a call back. Fill in the form at the bottom of this page, and we will call you back at a time that suits you.
Can I keep my current support workers?

Yes, you can! But they would need to come and work for Latrobe Community Health Service. We are recruiting staff, so if your support workers are interested, they can apply here. When you speak to our team, be sure to let them know you would like to keep using the same support workers.

How do I speak to someone who can help me with the paperwork to transfer my Home Care Package?
