Latrobe Community Health Service is one of the largest providers of aged, disability and carer services in Gippsland. With decades of experience, we are well placed to support your health and lifestyle goals.

Support for everyone

We provide information and advice, but we also encourage you to make important decisions about your own health and care. You know best how you’re feeling and what you’re capable of achieving.

We have three key support areas:

Aged care

  • Latrobe Community Health Service runs the Gippsland Aged Care Assessment Service (ACAS), which assesses eligibility for care in an aged care home and for other Commonwealth funded programs such as packaged care.
  • If you’re eligible for a home care package, we can help you coordinate the services you need to help you remain at home and in your community for as long as possible.
  • We also run planned activity groups that meet to enjoy doing things together in a friendly and supportive environment.

Disability services

  • At Latrobe Community Health Service, we are proud to provide planning and community connection services as a partner with the National Disability Insurance Agency.
  • We work to connect people living with a disability to informal and natural supports that are already available in the community. We provide assistance with the planning process, and ensuring plans are implemented effectively.
  • We also work with the community and service providers to build inclusion and awareness of the needs of people with disability.

Carer support

  • Latrobe Community Health Service gives carers of all ages the chance to take a short break from their role by taking on caring duties and providing opportunities to rest, socialise and recharge their batteries. We also provide fun activities for carers to get together and socialise.
